What You’ll Do

  • Conduct intake appointments and guide new patients through their care options 
  • Help identify appropriate mental and behavioral health services for each individual 
  • Answer new patient calls and follow up with potential patients 
  • Deliver excellent service and communication to both internal teams and external clients 

 

Who You’ll Work With

As a Care Coordinator, you’ll collaborate with: 

  • Clinical teams, including therapists, psychiatrists, and nurse practitioners 
  • Regional and administrative staff to ensure smooth onboarding and scheduling 
  • New patients and their families as they begin their care journey 

Your role is central to ensuring no one feels lost, overwhelmed, or alone as they begin care with  Elite DNA.

Who You Are

  • You hold a bachelor’s degree in Psychology, Social Work, or a related field 
  • You have at least one year of related experience (behavioral health or patient-facing preferred) 
  • You’re a great communicator who thrives in a fast-paced, people-focused role 
  • You’re empathetic, organized, and passionate about helping others access care
